Supported Living and Extra Care Supported Living is designed  to provide high level support services and flexible personal care as required to suit individual needs for comfort and companionship in a homely setting.

We actively encourage continued  independent living, enabling people to lead as full and active a life as they can according to their ability, by providing the means and assistance required to make informed choices about all aspects of life and living

Why is supported living right for me?
Our clients needs and desires have shaped the development of Supported Living over the past decade as an alternative to residential care, and came about as a result of  carefully listening to what people wanted in terms of preferences, comfort, security, support and care.

It's been designed by people like you who want to live their lives to the full in friendly, comfortable family sized houses.

We found that people wished to continue or develop a level of independence whilst also having the security of knowing that two trained members of staff were there 24 hours a day, 365 days of the year, plus a member of staff to attend to needs during the night should the need arise.  Another important factor was enabling individuals to retain access to their benefit entitlements (where applicable), and give them the option to decide who provides their personal care and support to ensure they remain in control.

There are currently 2 Extra Care Supported Living units in Okehampton at Lyndridge House and Moorview.  They are fully equipped for a variety of client needs including ceiling track hoists, wet room and a private consulting/beauty room.

What is Supported Living?

Supported Living is centred around you, enabling you to live your life the way that  you want to.  Your wishes, needs and desires can be catered for in terms of your  environment, care and support by offering you flexibility and choices of how you spend your income.

Supported Living is essentially 'group living' which offers a  combination of  privacy and companionship which enables you to maintain a feeling of independence whilst being in the company of others in a small and friendly environment, safe in the knowledge that trained staff are on hand day and night should you need them.

Lyndridge House and Moorview, the Extra Care units, have  space for 12 people and the homely environment includes en-suite bedrooms which can be personalised with your own furniture and belongings plus communal lounge, dining room, kitchen and lovely gardens.  Others range in size from 3 to 7 bed units.

It will be 'your home', and you  are  encouraged to play an active part in the  decision  making  process.  You can furnish and decorate your room to your own taste and even bring a pet with you (subject to the agreement of your fellow tenants). 
We can offer a range of daily activities and trips in our wheelchair friendly mini-bus, and encourage participation, but it is your choice how involved or active you wish to be. 

Wherever possible, we encourage  involvement with your family members, friends/advocates and strive to maintain your social links with past activities / interests as well as spiritual needs.

Locally sourced fresh fruit, vegetables and meat are available and delivered weekly plus we also have our own kitchen gardens and we support the Made-Well community project (www.made-well.co.uk).  Everything is home cooked daily to suit your tastes and requirements with a menu agreed with you.

Services

We offer a range of personalised services, under the guidance of the governments white paper 'Putting People First'.   These are all tailored to suit your individual needs which will be discussed fully in our assessment to find out about  your preferences.  Essentially, Supported Living is made up of core elements as follows:

The  Support element ensures staff are on-site 24/7 to facilitate enabling, financial and emotional support, well-being, all food, cooking, utility bills, travel, cleaning, laundry, shopping, activities etc.  Assistance could range from being on hand to help using the telephone or simply 'overseeing' the laundry.  The costs varies according to your individual Person Centred Plan and shared time according to how many other people you live with.

The Care element can be provided by Lyndridge Care or a person/agency of your choice, and if you already have a pre-existing provider, they can continue this for you.  Staying at a Supported Living unit is not dependent on receiving care, and you can choose to cancel the care contract without jeopardising your tenancy.    If you decide to use  Lyndridge Care for your care, we will provide qualified and trained Carer's to undertake all your personal care tasks, as required according to your assessed needs as part of your personal care and support plan.  The costs vary according to the provider chosen, but Lyndridge fees are from £15.00 per hour.

The Rent covers the costs for the sole use of your individual room, plus shared communal space in your home on an assured short hold tenancy for 12 months with a yearly rent review.  The cost is dependent on the accommodation that you choose, but is typically around £100 per week.

The Service Charge covers the building maintenance, insurance, gardens, equipment, service contracts, vehicles and such like, and is shared between the tenants, as are the utility bills.  You will be responsible for your own housing cost of living, and these will be dependent on your accommodation and how many other people you live with to share the bills with.

Funding & Support Services

Possible benefits available (subject to assessment & need) to cover these costs (for over 65's) could include pension, pension credits, attendance allowance and severe disability premium, housing benefit (where eligible) and winter fuel payments.  For under 65's could include  Income Support, Disability Living Allowance and Housing benefit.

Free assistance and independent advice is available to anyone considering a placement.  We  can  also  help  you  navigate  through  the  benefits  maze,  and assist  with paperwork to claim entitlements (where applicable).  You can call Devon County Council on 0845 1551005 or Department of Work and Pensions 0845 606 0265 for help too.
We can also help you with difficult topics including end of life and advance decisions.

Quality Standards

Both clients  and families are at the centre of all decision making where options are provided and you drive the decisions.  Your rights and choices are upheld, and we promote freedom of choice where your individual  expression is encouraged.  We offer equal opportunities to all our clients and do not discriminate on any grounds.

Quality Control

Care and Support services are regulated by the Care Quality Commission.  Processes are in place to ensure involvement in shaping day to day activities as well as ensuring high standards of service delivery.  These include  regular tenants meetings, landlord meetings, relatives meetings, care and support plan reviews, quality assurance surveys and spot inspections.  We also operate a  compliments and complaints system to ensure any problems are attended to promptly and effectively in the strictest confidence.

At Supported Living units, we are also able to offer the following services:

Day Care

For those wishing to use our wide range of facilities including assisted bathing/ showering, we can provide this at selected units. A special 'working week' (Mon to Fri) rate is available for permanent bookings (over 4 weeks).  Check with the main office for charges.

Respite Care

For those recovering from an illness, or after a stay in hospital that require support on a short term basis, we can offer respite care (subject to availability).  We are able to provide competent, trained staff, on duty 24/7 charged at our normal weekly rate.  However, please note that we do not provide nursing care.

Next stage

We understand that this is an important and life changing decision, and are able to offer a variety of options to ease the transition, depending on your requirements.

Supported Living Units are located in and around the Okehampton area plus Chagford and Crediton.

They are all completely individual, thereby offering a wide choice in terms of location, ambiance and 'feel', varying in size from three to twelve rooms.
